What you need to know before your first appointment — and what to bring.

New Patient Paperwork

When you come for your first appointment at Capstone, you will be asked to complete intake paperwork. This includes information about your health history, current concerns, insurance, and consent for treatment. Please plan to arrive 15–20 minutes early to allow time to complete these forms.

What to Bring

  • A valid photo ID (driver’s license, state ID, or passport)
  • Your insurance card(s)
  • A list of all current medications and dosages
  • Names and contact information for any other healthcare providers
  • Any prior psychiatric or medical records, if available and relevant
  • Emergency contact information

Forms You May Complete

  • Patient intake and registration form
  • Consent for treatment
  • HIPAA Notice of Privacy Practices acknowledgment
  • Insurance authorization and assignment of benefits
  • Release of information forms (if coordinating with other providers)
  • Financial agreement and sliding fee scale application (if applicable)

Telehealth Patients

If you are scheduled for a telehealth appointment, our staff will send you any necessary forms electronically prior to your visit. Please complete and return them before your scheduled appointment time so your session can begin on time.
