What Is the Integrated Health Home?
The Integrated Health Home (IHH) is a Medicaid-funded care coordination program designed for adults with serious mental illness (SMI) or substance use disorders. It takes a whole-person approach, recognizing that mental health, physical health, and social factors are deeply connected.
As an IHH provider, Capstone assigns each enrolled member a dedicated care coordinator who works alongside your entire care team to make sure all of your needs are addressed — not just your mental health.
Care Coordination
Your care coordinator helps schedule appointments, communicate between providers, track your health goals, and remove barriers to care — all at no additional cost to you.
Health Monitoring
We track important health indicators, help you follow up on preventive care, and support you in managing chronic physical health conditions alongside your mental health treatment.
Social Needs & Community Resources
We connect members with community resources for housing, food security, transportation, and other social factors that affect health and well-being.
Wellness Planning
Together, we develop an individualized wellness plan that reflects your goals and guides your care across all settings.

- Iowa Medicaid members
- Adults with serious mental illness (SMI)
- Adults with substance use disorders
- Those with complex, co-occurring health needs
Cost
The Integrated Health Home program is free for eligible Medicaid members. There is no additional cost or copay for IHH services.
