What Is Community Support?

The Community Support program provides targeted case management and community-based skills training for adults living with serious mental illness (SMI). Our goal is to help individuals develop the skills and support networks they need to live as independently as possible.

Targeted Case Management

A dedicated case manager works with you to coordinate your mental health, medical, housing, employment, and social service needs. Your case manager serves as your advocate and helps you navigate systems that can feel overwhelming.

Community-Based Skills Training

Services are delivered in the community — in your home, your neighborhood, and the places where you spend your time. We help with practical daily living skills such as managing appointments, cooking, budgeting, transportation, and maintaining housing.

Crisis Planning

We work with you to develop a personal crisis plan so you have a clear roadmap for getting help when things get difficult.

Coordination with Treatment Team

Your community support specialist works directly with your therapist, psychiatrist, and other providers to make sure all aspects of your care are aligned.

How to Access Services
